70 year old female Hx: Abdominal Pain O/E Pulsatile abdominal mass Findings: Aneurysmal dilatation of AA below celiac trunk until bifurcation (max: diameter ~ 12 cm). Above 5 cm indicates imminent rupture. Involved arteries include: SMA, Bilateral Renal arteries, IMA Aneurysmal dilatation of SMA / Common iliac vessels seen as well Eccentric thrombus seen in vessel (level? extent?) Whirlpool sign on color doppler False double/wide lumen seen. Possible dissection of intimal wall versus rupture contained by surrounding soft tissues. Flow velocity wasn't taken of abdominal aorta / renal arteries, though it should have been. CT Angiography to be advised. It's worth mentioning color doppler is meant for screening purposes only. Enough to mention +/- aneurysm w/ size +/- thrombus +/- surrounding fluid Detail is expected from CT angiography only.
