PUJ refers to a condition where the renal pelvis, the part of the kidney where urine collects, faces an obstruction at the junction with the ureter, the small tube that carries urine to the bladder. In simple terms, if the renal pelvis and the ureter are connected here, there’s a narrowing at this junction, causing various symptoms. Since this obstruction is located just outside the kidney, it can cause severe pain in the flank area. The pain may be constant or intermittent. Due to the blockage, urine flow is hindered, potentially leading to infections, fever, or even generalized sepsis. Patients might also experience symptoms like vomiting. The primary cause of PUJ obstruction is usually congenital, meaning it’s present from birth. However, in rare cases, it can occur due to previous surgeries. Diagnosis is typically done through a simple ultrasound, but advanced studies like CT scans, MRI, or DTPA scans may be required to determine the exact location and extent of the obstruction. In the past, the primary treatment involved open surgery, where a cut was made in the affected area, and the narrowed section was removed, allowing the normal tissues to be joined, thus relieving the obstruction. However, with advancements in technology, we can now perform this procedure using laparoscopy or even robotic surgery. These methods involve smaller incisions, faster recovery, minimal damage to the body, reduced need for painkillers, less bleeding, and quicker return to normal life.
