Dr. Rahul Kumar Rathore Chest Specialist at Charak Hospital Lucknow talks about Medical Thoracoscopy which lets the physician visualize the pleural membranes and take Biopsies to diagnose the cause of the undiagnosed pleural Effusion, and Therapeutic procedures to prevent further fluid formation. In India, the most common cause of Pleural Effusion is Tuberculosis (TB) and in the case of elderly people, it may be because of Cancer, although there are many other causes of pleural Effusion that exist. Sometimes it’s very difficult to diagnose the exact cause of the pleural effusion. The pleural fluid analysis only is not specific enough to conclude the diagnosis in most of the cases. In that scenario, Pleural Biopsy is a Gold Standard test for the diagnosis of Undiagnosed Pleural Effusion through Medical Thoracoscopy. It’s an Invasive day care procedure, which can be done in local or general anesthesia. A 40-Year-Old lady was admitted to charak Hospital, with complaints of breathlessness. She was diagnosed with Left-sided Pleural effusion with unknown etiology. The patient was started on Antitubercular treatment empirically, in view of a high index of suspicion for Tubercular etiology. Multiple times fluid was aspirated earlier at various other medical centers to give her symptomatic relief. But she was not responding to treatment and even the left lung was completely occupied by fluid leading to its collapse causing respiratory distress even at rest. At Charak Medical Centre, Thoracoscopy was done by our chest specialist Dr. Rahul Kumar Rathore, around 3-litre fluid was drained, a pleural biopsy was taken and a chest drain was placed to drain the remaining fluid. The patient improved symptomatically quite well after the procedure Biopsy turned out to be malignant on histopathology. So Medical Thoracoscopy is a very excellent tool in the diagnosis of undiagnosed pleural effusion and also has therapeutic uses in breaking adhesions and loculations in empyema and pleurodesis.
