The ABCDEF Guide for Iris Nevus Growth to Melanoma by Carol Shields

The ABCDEF is a simple guide to help clinicians assess the risk of malignant transformation of iris nevi. It was introduced in a study of 1611 eyes with iris nevus. Iris nevus growth to melanoma occurred in 3%, 4%, 8%, and 11% at 5, 10, 15, and 20 years after presentation. The following risk factors and their corresponding hazard ratios (HR) were determined through multivariate analysis: A - Age below 40 (HR=3) B - Blood in the anterior chamber [hyphema] (HR=9) C - Clock hours inferior (HR=9): located between 4 o'clock and 9 o'clock D - Diffuse configuration (HR=14) E - Ectropion uveae (HR=4) F - Feathery margins of the tumor (HR=3) Early diagnosis and treatment of iris melanoma is critical because larger tumors typically have more severe cytogenetics with a higher risk of metastasis. Compounding the issue, uveal (iris, choroidal, or ciliary body) melanoma metastasis is especially worrisome because there are currently no good treatments.
