Throughout the history, different therapies were used for TB. The role of each therapy varied over the time. Some therapies were standard at one time, became obsolete later on. On the other hand, new therapies emerged. In this lecture, we will discuss these therapies and we will see that some patients still benefit from old therapies used together with the new therapies. This fact emphasizes the individualized approach for patient's management. For the thoracic surgeon, tuberculosis is not a disease per se; it is a school from which valuable experience was gained and applied later in the management of other diseases. The treatment options for TB of the pleura and the lung discussed in this lecture include sanatorium therapy, collapse therapy, pleural drainage procedures, pulmonary resection and bronchoscopic dilatation of TB bronchostenosis. Drainage procedures include closed tube thoracostomy, rib resection and open window thoracotomy. Collapse therapies included induced pneumothorax, induced pneumoperitoneum, phrenic nerve crushing, plombage thoracoplasty, paraffin thoracoplasty, seven rib tailoring thoracoplasty and thoracomyoplasty. Pulmonary resectional procedures are discussed in detail emphasizing their advantages over collapse therapies. Most importantly, one has to remember that TB can simulate any pathology of the chest and the fact that the principal treatment is medical by using the anti-TB drugs.
