On November 1981, In Wookey Hole, Keith Potter and his OUCC team arrived at Wookey Hole for an exploration of its deeper, uncharted chambers. When they reached the third chamber, Keith secretly planned to search of a rumored underground river. he attempted a risky dive through a sump connecting Chambers 9 and 19. His excessive exertion caused disorientation, and he lost his mouthpiece. Despite struggling, he continued pushing forward, ultimately suffocating from lack of air and the cold, treacherous conditions.
