Though the presentation was unusual, his history was classic for a) hernia recurrence and b) subsequent mesh-related adhesion and tugging of his spermatic cord. He has pain when straining, laughing, standing, and sitting. He feels best when lying flat. A spermatic cord block temporarily resolved his testicular pain. Bear down imaging demonstrated the hernia recurrence. Surgery is the cure for this problem. The mesh must be carefully removed off the spermatic cord contents and the hernia recurrence must be repaired. Dr. Towfigh offered him an open anterior approach.
