Antiarrhythmics (Part 2) - ICU Drips

This lesson is part 2 of our discussion of anti arrhythmic medications. We continue on with the knowledge from the foundational information from the previous lesson and begin to talk about the different classes of anti arrhythmic medications. Each of these classes have different mechanisms of action from one another but there are multiple medications in each class that all share the common effect. We start off talking about class I medications, which uniquely are further subdivided in to 3 subclasses (Ia, Ib, Ic), again each with a slight variation on the mechanism of action. Essential though these medications are all sodium channel blockers, each though having a different impact on the action potential and the 2 drip medications that we talk about are Procainamide and Lidocaine. Next we talk about class II anti arrhythmics, which are essential our beta blockers, blocking the effects of epi and norepi on beta receptor sites, effectively inhibiting sodium channels. The main drip medication we talk about here is Esmolol. Next up is the class III medications which are the calcium channel blockers. We talk about selective vs non-selective medications as well as how there are technically 3 categories of these medications, the first being discussed in our previous lesson on vasodilators, and the final 2 being comprised of 1 medication each, Verapamil and Diltiazem. The next class is class IV, which is our potassium channel blockers. Again we show how the action potential is effected by these medications and the primary drip medication we talk about is a very common one called Amiodorone. The last class, class V is really a catch all class of medications that don’t really fit in any other class. The drugs here are not ones that we use in IV drip but are often commonly given specifically in the ICU so we do a quick discussion of Adenosine, Atropine and Digoxin. We finish this class and this lesson out talking about Magnesium and Potassium replacement which also can be grouped in here. Hopefully after watching this lesson including the previous lesson that you will have a much better understanding of the anti arrhythmic medications, from how they work and what they do, to some of the differences between each medication and how they specifically have their effect on our patients heart arrhythmias.
