In this video, I take you through an in-depth look at how I treat deep acne scars with a combination of advanced techniques including TCA Cross, CO2 pinpoint laser, and Erbium laser. My patient, who is of Asian descent, presents a unique challenge as treating skin with more melanin requires a nuanced approach to avoid complications such as post-inflammatory hyperpigmentation (PIH). Understanding the Fitzpatrick scale is crucial in ensuring safe and effective treatments tailored to different skin tones. TCA Cross (trichloroacetic acid chemical reconstruction of skin scars) is a powerful method that I use to target deep, narrow ice pick scars. By applying a high concentration of TCA directly into each scar, we stimulate collagen production from within. This helps fill and lift the scar over time, allowing the skin to rebuild itself naturally. However, due to the depth and severity of ice pick scars, multiple rounds of TCA Cross are often necessary. Each session contributes to a gradual increase in collagen and skin regeneration, making monitoring and timing between sessions essential. In addition to TCA Cross, I incorporate CO2 laser resurfacing for more targeted, deeper treatment. The CO2 pinpoint laser vaporizes the damaged top layers of the skin, promoting collagen remodeling and faster healing. It’s highly effective in addressing both deep and stubborn scars. However, when working with skin types that fall into the Fitzpatrick 4 to 6 range, such as my patient’s, it’s essential to adjust the settings to ensure we don’t trigger hyperpigmentation or any adverse reactions. To complete the treatment plan, I use the Erbium laser, known for its gentler touch. The Erbium laser is ideal for resurfacing shallower boxcar scars and refining the skin's overall texture. It precisely removes thin layers of damaged skin, stimulating collagen production with minimal risk of hyperpigmentation, which is particularly important for patients with more melanin in their skin. My patient’s treatment plan reflects the complexity of addressing multiple types of scars and balancing safety with efficacy. By combining these treatments and customizing them to fit her skin’s needs, we aim to achieve smoother, healthier-looking skin. It’s essential to monitor her body’s natural collagen response after each session and determine if additional rounds of TCA Cross or laser treatments are needed to maximize results.
