Actually it happens in the anterior wall and the front portion of the rectal wall in the lower portion. There will be small very superficial ulcers which will not be healing easily. It is supposed to be caused because of the rectal mucosal prolapsed. That anterior wall of the rectum when it projects into the anal canal. So because of the stools going over it and causing some sort of friction over it can cause ulceration and that may be causing some amount of bleeding and it's a part of the constipation. It's called Solitary Rectal Ulcer Syndrome. So there are different views on that. Some call it as because of the ischemia, that is blood supply to that part is less. But the present concept is because the intestinal mucosa, large intestinal rectal mucosa is projecting into the cavity and causing ulceration there. So there are different ways of surgically treating it. Also provided it is causing some problem either it's causing bleeding or if it is causing obstruction to the passage of motion then only we treat it surgically.
