http://flebologia.it Endothermal techniques have widely replaced surgical stripping in the treatment of saphenous vein incompetence and symptoms related. The Cyanoacrilate endovenous administration using a dedicated delivery system can obviate and improve some limitations of thermal ablation techniques, such as the need of tumescent local anesthesia alongside the vein, the use of postoperative compression stockings and the riskof paresthesia and other complications. The procedure is safe and effective, with an occlusion rate at 12 months over 90%. After having entered the saphenous vein by ultrasound-guided percutaneous access, the introducer sheath is advanced to the sapheno-femoral junction, and the catheter is positioned approximately 5 cm below the sapheno-femoral junction. The Cyanoacrilate glue is loaded into a syringe, and the syringe and the delivery catheter, previously flushed with saline solution, are connected to the dispenser gun. The catheter is then primed by advancing the glue with the gun up to the catheter tip. It's a walk-in walk-out procedure. No tumescent anesthesia is administered, being the procedure completely painless and comfortable. Following each injections of glue the catheter is immediately pulled back 3 cm and external compression on the vein is held with US probe and fingertips. No compression stocking is prescribed at the end of the procedure and patient can immediately resume normal daily activities.
