Pathologic 3 is best described as a dark, intellectual masterpiece that blends the philosophical depth and internal monologue of Disco Elysium with the plague-ridden medical drama of Vampyr. Its core gameplay loop, which revolves around manipulating time and gathering information to solve a central mystery. At a pure gameplay level, Pathologic 3 has shifted away from the survival horror (managing hunger and thirst) of the previous games and moved closer to a Medical Investigation RPG. It plays like a mix of Disco Elysium's internal investigation and Outer Wilds' time-loop mechanics, wrapped with complex doctor mechanics from Vampyr and the gothic lovecraftian setting of Sherlock Holmes and Sinking City.
