INTERDISCIPLINARY PROJECT: 3D PRINTING and POST-INFARCT INTERVENTRICULAR DEFECT

This is an interesting case of a 84 years-old male with history of type II diabetes, arterial hypertension, dyslipidemia. He was admitted to our Emergency Department for chest pain continuing for many hours. At ECG it was diagnosed with an infero-posterior STEMI. At transthoracic ultrasound the patient presented akinesia of the basal segments of the lateral and inferior left ventricle wall with evidence of an infero-posterior post-infarct interventricular defect with vast left to right shunt. The patient was immediately brought to our Cath-Lab. A chronic occlusion of OB and thrombotic occlusion of RCA pre-bifurcation was encountered with subsequent primary PTCA of RCA and good angiographic result At ventriculography the diagnosis of post-infarct interventricular defect was confirmed The patient was successfully stabilized with IABP and mild inotropic support. To plan an appropriate approach for surgical correction a model of the heart was obtained through 3D Print of the ECG-Gated CT. In the 3D print it is possible to see the large interventricular defect with contrast shunting from the left to right chamber.
