The descending limb is highly permeable to water but not solutes, increasing the tubular fluid's osmolarity as it descends into the medulla. In contrast, the ascending limb, particularly the TAL, is impermeable to water but actively reabsorbs sodium, potassium, and chloride via the Na⁺/K⁺/2Cl⁻ cotransporter. We explore the concept of the countercurrent multiplier system, which establishes the corticomedullary gradient critical for water reabsorption in the collecting duct under the influence of ADH. You’ll also learn about the countercurrent exchanger, involving the vasa recta, which preserves this gradient without washing it out. Clinical relevance includes the mechanism of action of loop diuretics like furosemide, which target the Na⁺/K⁺/2Cl⁻ transporter in the TAL and disrupt the ability to concentrate urine, leading to diuresis.
