I was referred this case by a restorative dentist from Miami to retrieve a broken implant screw fragment. The brand of implant is called Keystone Prima. The broken screw fragment threads appear to be burnished to the internal aspect of the implant fixture. There are a limited number of implant broken screw retrieval kits made specifically by each implant company for their own components. Alternative techniques of screw removal include slotting the head of the broken fragment with a 1/4 round bur and use of a flat head screw driver inserted into the slot to remove the screw fragment. Another technique includes the use of a 33 1/2 inverted cone in reverse mode to back out the broken screw fragment. There are risks involved with all screw retrieval methods. Screw threads are vulnerable to nicks, scrapes, and denting. Tap/rethreading devices can be carefully used to recondition the implant fixture's screw threads. Microscope use is imperative and should be used with all techniques to avoid unnecessary trauma. This series of 7 videos will take you through my steps for successfully removing a fractured implant screw and tap/rethreading the internal threads of an implant fixture.
