Too many people's written HPIs and presentations are difficult to follow because of this simple issue. This one change can turn it from being a confusing HPI to a clear, concise, and EXCELLENT presentation. I think the biggest reason people start to get confused on how to present the HPI is because of the sheer amount of information that we have to comb through in order to understand what is going on with the patient.
