Introduction Risk Assessment and Accident Investigation could be viewed as the before and after of any accident or incident. Effective Risk assessment is an absolutely key part of understanding what we are getting ourselves into and should highlight everything that could, potentially, go wrong. Get the risk assessment piece wrong and you could increase your need for an incident investigation. There are established methods for taking human factors into account in hazard identification and risk analysis and assessment, an adequate step-by-step understanding of the task or activity is key to understanding where we could cause ourselves harm or make mistakes. When we think about Human Factors in accident investigation we are interested in what acts the human did or didn't do that contributed to the accident. We are also interested in whether these acts were intended or were mistakes. This allows us to understand better how to prevent it happening again in the future.
