So tracking his whereabouts, Dean drives off to make things right and get Sam back. His car breaks down. He tries Sam last time, but no reply, because Sam had his phone on vibrate. It left Dean to walk the ten miles to town and to Sam's whereabouts. Sam saw his had a missed call from Dean, but once more, no voice mail, so he just shrugs and tosses it aside. During Dean's walk, he's reliving in the memories of he and Sam. It's only made him more determined to get to where he needs to be. His definition of home was wherever Sam was. When he finally gets there, he immediately starts apologizing, telling Sam he needed him and he was wrong. Begging for another chance. The ending of the video you see Sam hugging Dean, which is evident enough he's wanting another go at'em.
