It acts as a key anatomical landmark that divides the supramesocolic and inframesocolic compartments of the peritoneal cavity. The root stretches obliquely from right to left, beginning at the anterior border of the head of the pancreas, extending across the descending part of the duodenum, and ending near the anterior surface of the left kidney at the level of the tail of the pancreas. It lies approximately at the level of the second lumbar vertebra (L2) and measures about 15 cm in length. The structures related to the root of the transverse mesocolon from right to left include: • The descending part of the duodenum (second part) 🧪 • The head and body of the pancreas, especially the anterior surface 🩺 • The abdominal aorta and its branches like the middle colic artery, which courses within the mesocolon • The superior mesenteric vessels (artery and vein), which pass posterior to the root and supply the midgut 🩸 • The left kidney, specifically the anterior surface, where the tail of the pancreas and splenic vessels also lie close This root acts like a surgical corridor, guiding access to retroperitoneal structures during procedures like pancreatic resections, duodenal surgeries, and mesenteric ischemia repair. Moreover, infections or hemorrhage from retroperitoneal structures can track along the mesocolon, making it clinically significant in surgical and radiological planning.
