In cases of external branch of the superior laryngeal nerve injury following thyroidectomy, video laryngoscopy often shows subtle but telling signs. At rest, the vocal cords appear mobile and symmetric, which helps distinguish this from recurrent laryngeal nerve injury. However, during high-pitched phonation, the affected vocal fold may bow anteriorly due to impaired tensioning by the cricothyroid muscle. One key finding is deviation of the petiole of the epiglottis toward the same side as the nerve injury. This results from unopposed pull of the contralateral cricothyroid.
