In less than one percent of cataract surgeries, I get surprised by the tissues. When I examined this patient at the pre-operative consultation, the eye appeared normal other than the cataract. This patient had more of a cataract in this eye (the right eye) than the other eye (the left eye), but he reported no prior trauma that he could recall. The examination showed a normal anterior chamber depth, no phacodonesis, and I did not expect any significant challenges. But then I got surprised. The first clue was when I poked into the anterior lens capsule to start the capsulorhexis, it wrinkled extensively and it was evident that the zonular support was compromised. The catch is that this patient wants a toric extended depth of focus (EDOF) IOL which requires placement in the capsular bag as well as alignment on the correct astigmatic meridian and also centering of the EDOF beam shaping element. This case was very challenging and it took far longer than a more routine case. At the end, I was very happy with the outcome.
