Pancreaticojejunostomy (PJ) - Key Steps 1. Mobilization of Pancreatic Remnant: The pancreatic stump is carefully mobilized, ensuring clear visibility of the pancreatic duct. 2. Jejunal Limb Selection: A loop of the jejunum is brought up to ensure tension-free anastomosis and an appropriate point is chosen for anastomosis. 3. Suturing Duct to Mucosa (Modified Blumgart Technique): A posterior U stitch is placed horizontally in the pancreas, including the capsule, without involving the duct. The duct-to-mucosa sutures are then placed using fine absorbable sutures (6-0 or 5-0), typically starting at the 6 o'clock. After placing all sutures (usually 4-6), the jejunal mucosa is approximated to the pancreatic duct, ensuring a watertight closure. 4. Anterior Layer Suturing: An anterior row of U stitches is placed, reinforcing the anastomosis.
