We describe a primary posterior capsulotomy technique using a 25-gauge vitreous cutter to clear post-operative leftover cortical matter not the fibrotic PCO in patients with the non-dilating pupil. The posterior capsule is removed via a pars plana approach using a 25-gauge vitreous cutter; the removed capsule is round, well centred, and approximately 5.0 mm in diameter. This technique is safe in comparison to the anterior approach, but obviously, it is not the replacement of YAG LASER Capsulotomy. It minimizes the chances of IOL capsular bag dehiscence where IOL is being firmly gripped by the capsular bag.
