Dr. Ebraheim’s educational animated video describes the Positions for joint fusion arthrodesis. Arthrodesis means fusion or freezing of the joint. This is usually done to relieve joint pain after exhausting all options of nonsurgical methods including pain medication, therapy, injections and splints. It is an alternative to arthroplasty (joint replacement) when arthroplasty is not desirable or is not possible. Arthrodesis helps to relieve pain. This video will illustrate the optimal functional positions for the most common joint fusions. Shoulder •The patient is able to reach the mouth in this position. •Abduction 20 degree •Forward flexion 30 degree •Internal rotation 40 degree Elbow •If unilateral: 90 degree flexion •If bilateral: 110 degree flexion (one elbow to reach the mouth): Hand to mouth •65 degree flexion (one elbow for perineal hygiene) Hand/wrist MCP joint flexion: •25 degree index •30 degree long •35 degree ring •40 degree small PIP joint •40 degree index •45 degree long •50 degree ring •55 degree small. DIP joint: 10 degree Wrist Patients with non-rheumatoid arthritis of the wrist, 10-20 degree dorsiflexion is ideal because it allows position for power gripping. In patients with rheumatoid arthritis of the wrist, neutral or flexed position with 5-10 degree of ulnar deviation. Trapeziometacarpal •30-35 degree radial abduction •30-40 degree palmar abduction Hip •25-30 degree flexion •0 degree abduction •0 degree rotation Knee •10-15 degree flexion •0-7 degree valgus •10 degree external rotation Ankle •5 degree hindfoot valgus •5-10 degree external rotation •Neutral dorsiflexion Tibiotalocalcaneal •Neutral dorsiflexion •8-10 degree of valgus at the heel. 1st great toe MTP •15-20 of valgus in relationship to the metatarsal shaft •10-15 degree of dorsiflexion in relationship to the floor.
