TGD cyst is a fairly common midline neck swelling seen in children and some even present in adults It can be diagnosed in the clinic as it classically goes up in the neck on swallowing tongue protrusion. This is due to anatomical origin that has a close relationship with the thyroid and the tongue (hence the need to show you the earlier video). CT scan or ultrasound-guided biopsy are optional depending on the case. Treatment is needed as it is associated with a high risk of infection, with fistula formation and rare risk of cancer transformation. Traditionally surgery called Sistrunk operation recommended to remove the cyst together with the hyoid bone. However, now ultrasound-guided percutaneous ethanol option as a chemical ablation is a viable proven option for those who refuse surgery for whatever reasons. It can be done in the clinic without any operation theatre facility. It can be done under local anesthesia, quick and cheaper than surgery under general anesthesia, and does not result in surgical scar as in the traditional surgery. PEI has also been gaining popularity as a good alternative for TGDC treatment, which is associated with high feasibility, a high safety profile, and favorable treatment outcomes. Here in our centre, both the surgical option and the chemical ablation options are available. You decide !
