Understanding Dermoid Cyst: Symptoms, Diagnosis and Treatment #ultrasound @Dr.Saimakhan A dermoid cyst (also called a mature cystic teratoma) is a benign tumor that most commonly occurs in the ovaries but can also develop in other parts of the body. It is a type of germ cell tumor that arises from embryonic cells and contains a mix of tissues, such as hair, skin, teeth, fat, and even bone. Key Features of a Dermoid Cyst: Common Location: Ovaries (most frequent), but can also occur in the face, neck, spine, or brain. Composition: Contains mature tissue from different germ layers (ectoderm, mesoderm, endoderm). Growth & Symptoms: Usually slow-growing and asymptomatic. May cause abdominal pain, bloating, or pressure if large. Can lead to ovarian torsion (twisting of the ovary), which is a medical emergency. Diagnosis: Ultrasound – Shows a complex mass with echogenic areas, fat-fluid levels, or calcifications. MRI/CT Scan – Can help confirm the diagnosis if ultrasound findings are unclear. Treatment: Observation for small, asymptomatic cysts. Laparoscopic Cystectomy for larger or symptomatic cysts. Oophorectomy (removal of the ovary) in postmenopausal women or suspected malignancy.Treatment for Dermoid Cyst (Mature Cystic Teratoma) 1. Observation (Watchful Waiting) Small (5 cm), asymptomatic cysts in young women can be monitored with follow-up ultrasound every 6-12 months. If the cyst remains stable, no immediate intervention is needed. If it grows or causes symptoms, surgery is recommended. 2. Surgical Management Surgery is required in the following cases: ✔ Cyst 5 cm (risk of complications increases). ✔ Persistent pain, bloating, or pressure symptoms. ✔ Ovarian torsion (twisting of the ovary, which is an emergency). ✔ Suspicion of malignancy (though rare in dermoid cysts). Types of Surgery: 1. Laparoscopic Cystectomy (Minimally Invasive) Preferred for younger patients to preserve fertility. Only the cyst is removed, leaving the ovary intact. Shorter recovery time, minimal scarring. 2. Laparotomy (Open Surgery) Used for very large cysts or if malignancy is suspected. Requires a larger incision and longer recovery.
