Part 2 of my experience with a renowned vulvovaginal expert, also the story of how I went from a person with annoying provoked vestibulodynia, to frustrating chronic yeast, to disabling pudendal neuralgia. Part 3 will continue with the things that went wrong with this doctor, starting with the next procedure, the "scar band cutting." This installment discusses clitoral adhesions, balanitis, testosterone replacement, keratin pearls, smegma build up, the clitoral lysis of adhesions procedure, clitoral anatomy, the clitoral frenulum, and how my pudendal neuralgia symptoms began. 0:00 Issue #6 Doctor didn't fully examine me at intake, failed to diagnose clitoral adhesions and explain how important it is to clean under the prepuce while on testosterone therapy. 0:45 Issue #7 Prescribed systemic and local testosterone replacement therapy, which increases smegma production, without telling me about adhesions, smegma buid-up, or the importance of cleaning under the prepuce. Smegma can become trapped under the clitoral hood when someone has adhesions and get infected (known as balanitis in males), and form keratin pearls. 3:10 Issue #8 Didn't attempt conventional treatment of balanitis (topical steroid), or encourage conservative treatment of adhesions and keratin pearls (manual release); only told me to get lysis of adhesions procedure for $1,500. 5:03 Issue #9 Performed lysis in-office with no sedation, no pain medication, and inadequate administration of local anesthetic. My pain and shaking caused the doctor and nurse to have to use force and pressure on sensitive tissues to keep the operating area still and exposed. 5:47 My experience of what a lysis of adhesions procedure is like. 7:19 Post-lysis, beginning of right-side neuropathic pain from right clitoral frenulum (pudendal neuralgia). Article for non-medical folks on clitoral lysis (but the photos depicting it do not look like the photos of my lysis procedure, which involved much more swelling, stretching of tissues, cutting and bleeding). rachelrubinmd.com/post/new-research-on-cli...
