Abstract: The technique of fine-needle aspiration (FNA) has represented a great innovation in endoscopic ultrasound (EUS) procedures, especially in tumor characterization, because a pathological confirmation of lesions must be mandatory for therapeutic strategy in each case of a tumor. Submucosal lesions, lymph nodes, or pancreatic masses must be sampled by EUS-FNA in order to assess the benign or malignant nature of these lesions. The cytological diagnosis is thus critical for the differential diagnosis between chronic pancreatitis and pancreatic adenocarcinoma, as well as when a neuroendocrine tumor or metastatic lesion is suspected. Also, a tissue confirmation of malignancy is needed in advanced pancreatic cancer patients before referral to chemotherapy. For staging purposes, proof of malignancy is needed in case of M1 lesions (either metastatic lymph nodes or liver lesions, etc.), or to document vascular involvement that precludes surgery. The aim of this article is to briefly describe the clinical applications of EUS-FNA for the diagnosis and staging of pancreatic masses. This article is part of an expert video encyclopedia.
