Hello dear patients. Today we are going to review the case of opening of abscessed acne in the area of base of penis. A patient came to us with tender papule in the area of base of penis. You can see it on the picture. He used levomecolum ointment on his own without any effect. It was tender, particularly during touching. On observation that was a pustule or acne. What is pustule in this case? This is an inflammation of hair-sebaceous follicle. Sebaceous gland is quite tightly fused with hair follicle, it usually inflames after bacteria. If there's no rupture of pustule outside and the pus doesn’t get outside, then tender abscesses develop – collection of pus under the skin. In some cases it resolves on its own, in some cases it’s necessary to open it. Analyses for sexully transmitted infections were taken and as a result of them, gardnerella and ureaplasma were found in high concentration. There was some inflammation in the canal – leukocytes were 10-15. As a result, the parallel treatment of these diseases was prescribed; these diseases probably were the cause of immunity decrease and appearance of acne. However, in 2 weeks time, no resolution of this disease was noted, there was tenderness and a decision was taken to open up this neoplasm in order to let the pus out. Even the famous surgeon Pirogov said that where is pus – you should cut it. If it doesn’t resolve on its own and if inflammation continues to increase, as it was in this case, then this pustule was opened up, pus was removed and cavity was washed by disinfectant solutions. External antibacterial treatment was prescribed for healing as well as levomecolum ointment, zinerit solution etc. The therapy of ureaplasma and gardnerella was continued. As a result of that, gradually, within 10 days, a healing of this process was observed, the treatment of gardnerella and ureaplasma was finished and control analyses were taken. On control analyses, 3 weeks after treatment, this rash was completely resolved, leaving some scar and densification in the area of such neoplasm. So, if you see some pustule that doesn’t get out on its own, you shouldn’t press it out as this can lead to quite a significant abscess, pustulation of sexual organs.
