This case is about a 49 yrs old female presenting with right nephrostomy. The nephrostomy was inserted to alleviate kidney obstruction and consequent dilatation following oncological pelvic surgery. The report was of a distal ureteric obstruction about 2cm short of the vesico-ureteric junction Guidewire was unable to advance past what seems to be an absolute stop in the distal ureter. Ureteroscopy reveals an obstructed ureter and total obliteration of ureteric lumen. Retrograde contrast injection clearly defines the gap. The tip of the guidewire nearly bridges the short gap suggesting patency of the existing lumen. At this stage the ureter is cut with Holmium laser towards the guidewire as indicated by fluoroscopy. At the same time the guidewire is forcefully pushed from above towards the bladder. The tip of the guidewire is finally seen from bellow grasped with forceps and pulled through.
