URINARY BLADDER & URETHRA ANATOMY By #AnatomyHub Urinary bladder is the temporary store house of urine which get emptied through the urethra. ••External features 1-An empty bladder(tetrahedral in shape) 2-A full bladder (ovoid in shape) 1-An empty bladder *Apex; directed forwards ➡ *Base or fundus, directed backwards 🔙 *Neck, which is lowest and most fixed part of the bladder *Three surfaces, superior, right( 👉) and left ( 👈) inferolateral. *Four borders, two lateral, one anterior and one posterior. 2-A full bladder *An apex, directed upwards (👆 ) the umbilicus. *A neck, directed downwards( 👇). *Two surfaces,anterior and posterior. ••Relations *In male apex is connected to umbilicus by the median umbilicus ligament *base -In female it is related to the uterine cervix and to the vagina. -In male the upper part of the base is separated from the rectum by rectovesical pouch (👝 ). *The neck is the lowest and most fixed part of the bladder, it lies 3 to 4 CM behind the lower part of the pubic symphysis. -in males, smooth muscle ( 💪) bundle surround the bladder neck and preprostatic urethra. -In females, neck is related to the pelvic fascia which surrounds the upper( 👆) part of the urethra. *superior surface -In males, it is completely covered by peritoneum, contact with sigmoid colon and coils of the terminal ileum. -In females, peritoneum covers the greater part of the superior surface, except for a small area near the posterior border, which is related to the supravaginal part of the uterine cervix. *Inferolateral surface -In males, each surface is related to the pubis, the puboprostatic ligaments, the retropubic fat, the levator ani and the obsturator internus. -In females, the relations are same, except that puboprostatic ligaments are replaced by the pubovesical ligaments. ••True ligaments of the urinary bladder -In females urinary bladder *lateral true ligament *Pubovesical ligament *posterior ligament *lateral ligament of rectum *median umbilical ligament is the remnant of the urachus. -In males *lateral puboprostatic ligament *medial puboprostatic ligament *median umbilical ligament •False ligaments -median umbilical fold -medial umbilical fold -lateral false ligament -posterior false ligament ••Interior of the urinary bladder *In an empty bladder, the greater part of the mucosa shows irregular folds due to its loose attachment to the muscular coat. *In a small triangular area over the lower part of the base of the bladder, the mucosa is smooth due to its firm attachment to the muscular coat, this area is know as the TRIGONE of the bladder . *Trigone is directed downwards (👇) and forward( ➡ ) The internal urethral orifice, opening into the urethra is located in trigone . *Ureters open at the posterolateral angel of the trigone. •opening are 2.5 cm apart in the empty bladder . •5 cm apart in a distended bladder . *A slight elevation on the trigone immediately posterior to the urethral orifice produced by the median lobe of the prostate, is called the uvula vesicae. *The base of the trigone is formed by the interureteric ridge or BAR OF MERCIER produced by the continuation of the inner longitudinal muscle coat of the two ureters.
