In this article we want to answer, what does a high dhea sulfate in a female mean? This can come up on labs in a lot of females with PCOS. This leaves many with PCOS asking what does this mean and what can I do. This is not necessarily dangerous in and of itself but it can help us understand what could be going on in your body and more importantly why. There are additional lab studies to look at. We will look at the things that stimulate DHEA-s to be produced and also how it is being broken down. Let's look at what a high dhea sulfate and a female means [Music] so what does a high dhea sulfate and a female mean what does that tell us about what's going on in their body and if it shows up in your labs what does it actually mean what can you do about it things like this so that's we're going to be addressing and as with any system hormonal system etc you want to look at what's coming into that system and how it's how the the substance is being eliminated so what stimulates the production of the dhea sulfate and then how is it taken out of the system or broken down so with dhea the main source of the dhea is being produced from the adrenal glands and it's the innermost part of the adrenal glands that's producing that so what causes the stimulus for the dha to be produced mainly that comes from a hormone called acth and the acth it comes from the pituitary gland and it stimulates the adrenal glands mostly to produce glucocorticoids the acth is being stimulated when there's a fight or flight response so you get some activation whether you're in a car accident or someone's chasing you you're going to get more acth produced and that's going to stimulate the adrenal glands to produce cortisol along with that the innermost part of the adrenal glands are going to be stimulated and you're going to get increased production of dhea now there's always some background production of the acth and therefore cortisol so what we want to look at is what's you know above normal what's above your baseline level that could be a cause for increased dhea when we're looking at cortisol levels there's a few different ways to do that and the morning cortisol is a good option but when that's seemingly not reflective of what seems to be going on you can also do a hour cortisol to evaluate this so a spot check in your court am cortisol levels may show normal but if you look at the total load over hour period you may it may show like above normal levels so those are two ways that we typically use there's also salivary cortisol which is another option we don't tend to use that one as much so the question then is you have high dha sulfate do you also have high cortisol and how do you measure that so you got to make sure you might want to do it several times and look at different angles if you're not if it's not correlating with the high dhea so so this i know for sure can cause elevated dhea levels okay so what happens if you actually don't have high cortisol you've done several tests everything looks normal well then we want to look at what could be going on with the breakdown of your dhea sulfate now dha sulfate is turned into androstenedione and then testosterone there are some alternative pathways for this uh could be turned into estrogens as well they they're all dependent on one enzyme and that enzyme is called three beta hydroxy steroid dehydrogenase and it makes that conversion fairly readily but it is dependent on a few different things so if there is a high amount of androgen receptor and estrogen receptor activity then this enzyme can be down regulated so if you have high testosterone or high estrogen you may get a down regulation in this enzyme therefore raising the dhea sulfate or not allowing it to be converted into its normal path the other thing that regulates the three beta hydroxy steroid dehydrogenase is the cofactor nad your body is always keeping is trying to keep a relatively higher amount of nad to nadh and when the ratio flips when you have more nadh it kind of slows down the overall metabolism and you see this with in people with diabetes so your body's always in flux but people with insulin resistance and diabetes typically have low nad. with pcos often have insulin resistance DHEA-s
