With shrinking insurance reimbursements and rising costs, the private practice model of one on one care paid by insurance is facing ever-growing challenges. In light of these developments, physiotherapists are called to accept innovative solutions in order to survive and thrive. We need to change before change is thrust upon us. One promising approach is the team based physician clinic model, also called the 'dental team model'. This team based model allows physiotherapists to delegate routine tasks to skilled assistants, freeing them up for more complex and high-value work, like patient education and manual therapy. High volume clinics, though often maligned, can provide quality care by utilizing standardized protocols and effective workflows that maintain quality patient outcomes. Most physiotherapy patients improve with simple treatments, while a some patients need more complex, team-based care. This is the 80/20 rule. Effective physiotherapy involves empowering patients through active treatments, education, and simplified approaches. Working with a team, including skilled assistants, can boost efficiency and free up time for more complex tasks. Working together will allow physiotherapists to provide patient care with greater ease, efficiency, and not burnout or rage quit. A busy physiotherapy clinic can deliver quality care by using efficient systems and teamwork, inspired by the physician's clinic model. This approach streamlines workflow, frees up clinicians' time, and prioritizes patient care. Ongoing communication, education, and improvement help drive better patient results. After 30 years as a physio, I have come to realize that the traditional one-on-one model may not be sustainable long-term, especially with declining insurance reimbursement.
