Prasanna Venkataraman, Anahita Shroff1, Nivetha Gandhi Glaucoma Services and 1Department of General Ophthalmology, Aravind Eye Hospital, Chennai, Tamil Nadu, India Abstract Background: Non-penetrating deep sclerectomy (NPDS), in contrast to trabeculectomy, is a filtration surgery developed to lower IOP without entering the anterior chamber. As there is no penetration, there is a controlled filtration of aqueous, making it a more reliable and safe surgical option than trabeculectomy, in terms of lesser hypotony-related complications. Purpose: To illustrate the surgical steps of NPDS and add emphasis on the management of common intraoperative complications. Synopsis: The surgery involves the creation of a superficial and deep scleral flap, excision of the deep scleral flap, peeling of the inner wall of Schlemm’s canal exposing the Trabeculodescemet’s window, and allowing controlled aqueous outflow. We will walk through the difficulties faced during each step and address the same with appropriate management strategies. The video will serve as a teaching video guide for beginners and also help the masters to fine-tune their surgical skills. Highlights: The video helps surgeons master the surgery by walking through the crucial steps and demonstrates how to handle the intraoperative obstacles efficiently to achieve a good postoperative outcome. NPDS has a steep learning curve, and we aim to simplify the same with our video guide.
