You are reviewing the overnight patients on the clinical decision unit. They are being presented by a capable but inexperienced junior doctor. Go through the cases with the junior doctor, ask any relevant questions and agree an appropriate management plan. Provide appropriate teaching on the cases. This is a regular task in most EDs where patients who have been placed in the clinical decision unit (CDU) must be handed over and reviewed. With any handover of patient care there is the possibility of information being missed and patients not receiving appropriate and timely treatment. Doctors performing this task must be vigilant to ensure management plans are reviewed and modified as necessary. Required tasks and management plans can then be implemented as necessary whether they involve further investigations, referrals to in-hospital specialties or discharge planning. As with any ward round it also provides a valuable teaching opportunity. Remember in the exam to look carefully at the pie chart to see how the marks are distributed.
