Schwannomas are relatively common benign nerve sheath tumors that arise from either cranial or peripheral nerves. The majority of cranial nerve schwannomas arise from the eighth, fifth, and seventh cranial nerves in decreasing order of frequency. Most schwannomas have a characteristic MR appearance on T2-weighted imaging, with heterogeneous hyperintensity relating to the ratio of compact (Antoni A) cells to loosely arranged (Antoni B) cells, with avid postcontrast enhancement typically seen. The differential diagnosis depends on the location of the lesion, and there are certain distinguishing features that are helpful in this analysis. The article in the current issue of RadioGraphicsby Dr. Aaron Skolnik and the group at the University of Pennsylvania highlights the characteristic features of cranial nerve schwannomas and the role of imaging in treatment planning for affected patients.
