Dr. Harris: I'd like to talk a little bit about the direct anterior approach to hip replacement surgery. A lot of people will try to pretend that this is something that's brand new. It really isn't. The first mention of this in the English language literature was May of 1950 and if you read the article, the surgical technique is the basis of the direct anterior approach that we do today. Now any sort of surgery, if you think about it really critically, does some damage. You make a decision that's, leaves a scar in the skin. So how you close the incision to make it look nice matters, as well as using the incision to get into the space you need. In the same sense, the surgery of the direct anterior is designed to do the minimal amount of damage to the soft tissues to get in and get out of the joint. So to get in the joint with this approach, you do not need to cut any muscle. You do not need to detach any muscle from their connections to the bone. So the only thing which is cut is the capsule to the layer surrounding the joint itself, so that you can get into the joint and then the rest of the bone work that needs to be done.
