This patient had cataract surgery with an Apthera IC-8 IOL (Bausch & Lomb) months ago in order to give an extended depth of focus (EDOF). Like all EDOF IOL designs there is a compromise to be made and in this case that means loss of some of the incoming light which is blocked by the black ring on the optic. The center aperture is 1.36 mm in diameter while the outside of the black ring is 3.23 mm wide. When the pupil is larger than this then additional light will go around the outside of the black ring and that may contribute to dysphotopsias. This patient was unhappy with this IOL and wanted an IOL exchange for a monofocal IOL with a full, unblocked optic. Normally to explant an IOL, I prefer the twist-and-out method but for this case, I elect to cut the IOL in half -- why? Please comment below about how you would do this IOL exchange and if you have had to explant any pinhole IOLs.
