A very wise professor once told me, "Every surgery is just another opportunity for you to face grief." That grief can be from patients with unrealistic expectations like one I saw yesterday who says that his vision is terrible about 1 week after cataract surgery though he is -0.25 spherical and sees 20/20 while his pre-op BCVA was 20/80 from cataracts with a refraction of +1.50 -1.00 x 90. But that grief can also be from unexpected complications during surgery such as this case where a pupil expansion ring was helpful during cataract surgery but when it was removed, it inadvertently damaged ocular tissues. What is your next move?
