Ortho-keratinized odontogenic cyst (OOC) is a developmental cyst of odontogenic origin. It is a distinct entity from odontogenic keratocyst (OKC) and shows a less aggressive behavior. Key Features: • Origin: Derived from dental lamina remnants. • Histology: The cyst lining is orthokeratinized stratified squamous epithelium (shows a well-defined granular layer and orthokeratin on the surface). • Epithelium thickness: Uniform, usually 4–9 cell layers thick. • Inflammation: Minimal or absent unless secondarily infected. • Clinical features: Often asymptomatic, may present as a slow-growing jaw swelling. • Location: Most common in the posterior mandible. • Radiology: Unilocular radiolucency, well-defined borders. • Behavior: Less recurrence rate compared to OKC. • Treatment: Enucleation with minimal recurrence.
