Dec 2021: Professor Reinstein describes the evolution of sizing formulae for choosing the ideal size of the implantable collamer lens (or implantable contact lens or ICL). He shows how ICL sizing from white-to-white was improved by using UBM based sulcus-to-sulcus measurements, then adding the sulcus-to-sulcus-lens-rise. He then describes the latest and most predictable ICL sizing formula to date: a formula that includes 2 parameters never before included: the ciliary body inner diameter, and the scotopic pupil size. He then further refines the science of ICL sizing by proposing that rather than the formula suggesting one ideal lens, the formula should provide the predicted vault for each of the ICL sizes available, thus allowing surgeons to further customise the choice of what ICL to use. This new approach minimises the chances of needing an ICL Exchange surgery and maximises the chances of the eye ending up with the size that is most likely to provide long term safety.
