Chronic Rhegmatogenous Retinal Detachment

Shared by Dr. Aureliano Moreno from Tijuana México This a case of inferior chronic Retinal Detachment treated with primary vitrectomy. Patient had a small peripheral horseshoe-shaped break inferiorly and another one larger, superiorly in the mid periphery. The presence of multiple choroidal detachments in this case makes the surgery more challenging and more dangerous, especially during peripheral vitrectomy. During the shaving of vitreous base, the surgeon carefully removes all the vitreous around the tear flap. After diathermy, its important also to remove the rolled and thickened edges of the breaks. Despite the chronicity of the case, the retina was mobile, and it was possible to reattach the retina performing only Fluid air-exchange, without using PFCL. In the end of the surgery, laser photocoagulation was applied only around the breaks and silicone oil was implanted.
