This video will demonstrate how to document a discharge summary in Meditech and compete the discharge process. If discipline is being discharged (not being replaced) – DO NOT say yes to initiate discharge OR change patient status to Discharge on patient status page. This will initiate an AGENCY discharge – this is for RN use only. • Record Non-Visit Documentation (reason – discharge) o Document on Assess Discharge Management Care step (type “discharge” in care step search box). Document patient status/function as of the last DOS and the reason for discharge in the Notes field of this care-step. o Enter Communication to all team leads and supervisor indicating last DOS and reason for discharge. o TIP – if discharging due to insurance denial, document reason for discharge as “Discharge as per MD order”. We will only discharge with approval from MD. • Add/Update Orders task - if patient is staying on program, but just your discipline is being discharged. o Care Plan page – mark all discipline Care Needs, Goals and Care Steps as Not Needed or Met o Resource Plan page – Change resource Class to Discharge o Patient Schedule page - Remove your name from visits on the schedule so they do not appear in Confirm My Day and reassign to DC ORDERS, Needed. If patient will continue with service but you will no longer provide services (you are being replaced): • Record Non-Visit Documentation (reason – Discharge) o Document on Assess Discharge Management Care step (type “discharge” in care step search box). o Enter Team Communication to all team leads, supervisor and scheduler AND Care Coordinator (Case Placer) indicating last DOS and reason for discharge. • Add/Update Orders task o Care Plan page – do not make any changes o Resource Plan page – Leave resource class status as active – remove your name as lead clinician o Patient Schedule page – Remove your name from future visits and enter REPLACEMENT, NEEDS as the employee.
