A white or dense posterior subcapsular cataract often leave residual dense fibrotic material that adheres to the posterior capsule. This material can be very resistant to polishing away and often reduces the postoperative recovery of vision. More often than not an early posterior capsule YAG laser is required for full rehabilitation of vision. Alternately, I have used a posterior capsulorhexis to alleviate these issues. One difficulty with this technique is intraoperative stability of the lens at the conclusion of the case. A posterior optic capture or button holing technique, as advocated by Menapace, provides a very stable lens position and is demonstrated in this video.
