近視雷射手術後的白內障手術(Phacoemulsification and IOL power calculation in eyes that have undergone LASIK)—案例分享 新眼光眼科診所 蕭裕泉醫師 http://www.asianewvision.com.tw 微小切口白內障手術及植入五合一多焦點人工水晶體 (Phacoemulsification, micro-incision cataract surgery MICS) (ACRYsof IQ ReSTOR multifocal TORIC IOL +3.0D) 病患王◎◎約60多歲女士，於20多年前近視約850度，因為她很討厭戴眼鏡、覺得戴眼鏡很麻煩也不好看、不方便，所以在早期近視雷射剛引進台灣時，患者就為了不想配戴眼鏡而去接受近視雷射手術治療，手術後效果良好且視力也維持了近20年，但近一年來由於視力日漸模糊且近視度數一直增加至約1000度，最佳矯正視力僅剩0.1左右，起初患者以為是手機使用過度造成，但經休息後仍未見好轉，後來上網遍尋台北市的眼科名醫，於是來到我們診所找蕭醫師檢查，發現原來是白內障日益加深所造成，經過診所專業護理人員檢查及詳細說明過後，患者決定在這次白內障手術的同時，也一併解決她後來因白內障所造成的近視、散光、及因年紀所造成的老花眼問題，於是在本診所做右眼的微小切口白內障手術(Phacoemulsification, micro-incision cataract surgery MICS)及植入五合一多焦點人工水晶體ACRYsof IQ ReSTOR multifocal TORIC IOL +3.0D，術後效果良好，裸視視力可達1.0。 矯正一：白內障治療cataract 矯正二：矯正近視/遠視問題myopia/hyperopia 矯正三：矯正老花的多焦點設計presbyopia 矯正四：矯正散光astigmatism 矯正五：非球面鏡片細膩矯正高階球面像差aspheric 手術步驟 手術病患術前簡介 Chart review. 為病患撐開眼睛，準備開始手術。 Topical anesthesia with 4% xylocaine，cutting the eyelashes，Bata-iodine irrigation 2.2mm corneal incision is made by microsurgical knife. 於前房注入人工玻璃體，再用前房鑷撕開水晶體囊，便開始水晶體乳化手術。 Viscoat is injected into anterior chamber. The anterior chamber is inflated with dispersive OVD. A small paracentesis in then placed 90∘away from corneal incision. Central linear cut in the anterior capsule, using a cystotome needle. Grasp the free edge of the anterior capsule with Ultrata forceps, and the flap is carried around in a circular manner. Hydrodissection to separate the peripheral cortex from the underlying capsule. Phacoemulsification to remove the nucleus of the cataract. 沖洗剩餘的白內障皮質部份。 Using an irrigation/aspiration hand piece to remove the peripheral cortex of the cataract. 再次確認病人資料與欲植入的人工水晶體度數，接著植入人工水晶體，並調整散光的軸度。 Viscoat is injected into the capsular bag. The IOL is folded and passed through within the capsular bag. Roate the multifocal toric IOL to correct right axis and stromal hydration performed for corneal incision. 使用自動人工水晶體植入器AutoSert® IOL Injector植入五合一多焦點人工水晶體ReSTOR TORIC。並調整人工水晶體的散光軸度。 Injected the 「ACRY sof IQ ReSTOR TORIC IOL」with AutoSert® IOL Injector .Then ,rotated the IOL for adjusting the astigmatism. 將剩餘的人工玻璃體吸除，並於手術結束前給予消炎藥水及抗生素。 Miostat is injected into anterior chamber. Anterior chamber is irrigated with BSS.
