Intraoperative MRI of a Live Brain Surgery Dr.Roopesh Kumar #iMRI #mri #DrRoopeshKumar #braintumor When we perform a surgery to remove the brain tumor, “have we removed it entirely or have we left a part of it?” such a doubt can arise at times. So if it is a high grade tumor, the differentiation between the brain and tumor would be easy. Whereas, if the tumor is of a lesser grade, the differentiation between the brain and the tumor would be lesser. There would also be another doubt if the tumor was removed entirely or is a part of it left? Especially, lower grade tumor has to be removed completely because if we do not remove a part of it, there is the possibility for it to turn into a higher grade tumor in the future. Then it needs to be continuously monitored. So as much as possible, it has to be removed. Earlier, once the operation is over, the next day we take a CT scan to ensure everything is fine. We do not do that nowadays. As far as the CT scan is considered, there is not much clarity of the tumor in the CT scan. MRI is better. But can an MRI scan be taken as soon as the surgery is over? Earlier that was not possible, but now the answer is yes. Intraoperative MRI & Perioperative MRI, that is what we call it. In India, only in 3 or 4 centers the MRI machine is in the operation theatre itself. But that is too much of an investment in equipment. That is why 2 or 3 government hospitals and premiere institutes like AIIMS and NIMHANS only have it. So what do we do? Perioperative MRI. That is, once the operation is over, before the patient can recover from aneasthesia we shift the patient to the MRI room and take the MRI. After taking the MRI, if a part of the tumor remains and is not cleared, then we will immediately bring the patient back to the operation theater. Everything will be ready then and we remove the remaining tumor. So, perioperative MRI is being used lately. The benefit of this system or practice is that the residual tumor, that is to know how much of the tumor is left as such, is an important factor. By doing perioperative MRI the chances of leaving residual tumor is very less, as we check it. Before complete closure, we scan and check. If there is a residual tumor, we can bring them back to the operation theatre and remove the residual tumor. By doing such things, the oncological outcome will be improved for the patient.
