High flow petrous Dural AVF : strategy and play of catheters

Treated a young man with a high-flow dural AVF, fed by meningeal and pial arteries, with an aneurysmal change on the anterior inferior cerebellar artery. The key was forming a strategy; using two microcatheters and the pressure cooker technique. Distal access into the middle meningeal artery and wire changes allowed us to position the micro-catheters close to the fistula. Once in place, the procedure was straightforward. I believe that in dural AVFs, the strategy is crucial, and achieving a cure in one attempt with Onyx or other DMSO-based materials is essential.
