Inguinal hernia in a child is due to an open canal which causes a defect in the muscle through which the intestine can come out. Lot of parents ask if it can be medically treated and if surgery is required. If your child has inguinal hernia then surgery is a must. Through surgery we will close the defect and prevent the intestines from coming out. Q2: Can we wait for surgery? Inguinal hernia in a child will never go away. If we wait longer, as the child cries and strains the hernia will get bigger. There is a possibility that one day a part of intestine can get stuck leading to obstruction and emergency surgery will be required in those children. Hence instead of waiting longer its better to plan the surgery at the earliest and at your convenience. Q3: What are the options for hernia repair in children? Hernia in a child can be repaired through either open surgery which is called herniotomy or through laparoscopic method. In open surgery, the surgeon makes a small cut(incision) in the area of the hernia. The surgeon puts the loop of intestine back into the abdominal area, closes the defect and stitches the muscle together. Open surgery(herniotomy) is preferred because it is a day care surgery, shorter duration of surgery which lasts for around 30 minutes, less invasive anaesthesia, scar is in the lower abdomen and sutures are within skin which gets absorbed and scar is small and gets hidden in the panty line. Laparoscopic surgery is reserved for certain special circumstances. Q4: When is laparoscopic hernia repair advisable? Laparoscopic hernia repair in children requires more invasive anesthesia. There will be 3 small cuts in laparoscopic surgery whereas in open surgery there will be one small cut in the lower part of the abdomen. Hence, we do the laparoscopic surgery in certain special cases such as: 1. Hernia is present on both sides 2. Hernia is stuck which is called obstructed hernia 3. Hernia in children with disorder of sex development 4. Hernia in a child who was operated in the past and there is swelling at the same site where surgery was done In all these cases we prefer to do laparoscopic surgery over open surgery Q5: What is the post-operative recovery? Hernia surgery in a child takes around 30 minutes. During surgery for hernia, your child will be given medicine to put him or her to sleep(anaesthesia). Once surgery is done child will be shifted to recovery room and will be monitored in this recovery room for another 30 to 60 minutes. Orals are started 2 hours later when the child is awake. Liquids is given initially and normal diet can be given by the same evening. There is no restriction of activity for the child after surgery. They are allowed to play, run and jump on the same day. Child will be discharged on the same evening after surgery. There will be a small dressing at the site of surgery which will be removed at follow up visit in the OPD. All stitches are inside the skin and no stitch removal is required. The post-operative recovery is same for open and laparoscopic surgery.
