The septate uterus is a commonly misdiagnosed, undiagnosed, and neglected condition, which many doctors and experts of the field believe to be a relatively harmless factor in infertility, ectopic pregnancies, chemical pregnancy and recurrent miscarriage. What is a septate uterus? A septate uterus is the most common form of uterine malformation, meaning uterus with abnormal shape and size. For this form of uterine anomaly, there exists a septum consisting of primarily connective tissues which is protruding towards the inner cavity. It can vary in severity depending on the degree of the septum: mild, medium & severe septate. This congenital uterine abnormality originates in a defect the fusion of the two Mullerian Ducts during embryogenesis. Although it does not cause any symptoms, it can lead to high risk of pregnancy losses, recurrent miscarriage, and ectopic pregnancies: all of which occurs within the first trimester. Diagnosis is done via the highly effective Hysterosalpingograph, where an Iodine Dye is inserted into the uterine cavity to capture the morphology. How septates cause pregnancy complications are that for miscarriages that occur within the first trimester, the implantation can occur directly on the septum as the embryo can encounter this directly after exiting the fallopian tube. This will lead to malnourishment and lack of oxygen for embryonic development, leading to an early miscarriage. Depending on the severity, this miscarriage can occur within 2-3 weeks of gestation, otherwise known as a chemical pregnancy. For ectopic pregnancies, the septate can cause the altering of movement of the embryo during implantation and lead it to the opposite fallopian tube, causing the most common Tubal pregnancy with its own potential life-threatening complications. This can happen regardless of septate severity. Mild septates are not emphasize for treatment Many times, doctors only believe surgical managements should only be recommended for only severe septate despite the surgical hysteroscopic intervention being relatively simple and minor. There is a lack of awareness that even mild septate can equally cause high risk of pregnancy failures and ectopic pregnancies. They do not remove septums that are medium or mild, as they consider it to unproblematic and does not contribute to any potential pregnancy complications.
