It courses along the medial border of the psoas major muscle, lying anterior to the sacroiliac joint, and travels under the inguinal ligament, where it becomes the femoral artery — the primary artery of the lower limb 🦵. Before this transition, it gives off two crucial branches: • The inferior epigastric artery 🧬, which ascends to supply the anterior abdominal wall and forms an important anastomosis with the superior epigastric artery, and • The deep circumflex iliac artery, which runs laterally along the iliac crest to help supply the lower abdominal wall 🧠. Clinically, the external iliac artery is a key landmark for vascular surgeons during procedures like femoral artery catheterization, and it may be involved in atherosclerotic disease, causing limb ischemia 🚨. It is also important in renal transplant surgeries, where the donor renal artery is often anastomosed to it.
